[Houston Social Forum] sign up sheet

I created the attached sign up sheet. Let me and Garry
() know if you want to
add/change something. I figure if people say yes to
the mailing list question, we can subscribe them
rather than overwhelming interested parties with
details about how to sign up.

Garry, can we get something like this on the website
in the next week or so? there is database capability
with our website?
